How to Update Your Provider Contact Information
Medicare providers can confirm that the CERT Documentation Contractor has
the most up-to-date information by accessing the CDC
website
. On the “Provider Address Directory Page” the provider may enter
the Medicare Provider ID to verify their contact information. If changes are
needed to the listed information, providers can contact the CERT Documentation
Contractor either by email via their website or by phone at (301) 957-2380 to
report the changes. If the search does not locate contact information for a
provider, the provider is encouraged to submit the information to the CERT Documentation
Contractor. Providers are encouraged to include the name and phone number for
a contact person who is familiar with the CERT process and can respond to the
CERT requests by sending the appropriate documentation.
All revisions involving contacts and/or addresses and phone numbers will not
affect the information on record with the CMS provider enrollment office or
the Medicare Part B/Part A/DMERC contractors' provider vendor files.
