How to Report Medicare Fraud

Medicare is very interested in hearing from you! To report suspected Medicare fraud or abuse, please:

• Contact the National Fraud and Abuse Hotline - 1-800-447-TIPS (1-800-447-8477) OR
• Contact your Medicare Intermediary or Carrier. Their phone number can be found on your Medicare Summary Notice (MSN). You will reach a customer service representative and you can report your concerns to them. OR
• Provide us with the following information via e-mail to ugsadmin@cobalt-corp.com and we will contact you by phone for a private discussion of the concern(s) you have:
