Recommended Documentation for Appeal

• Completed Redetermination Request Form and/or a signed letter requesting appeal.
• Appointment of Representative form (AOR) with additional documentation if needed. See "Completing an Appointment of Representative or Waiver of Payment Form" to be sure an AOR is required and also when additional documents are needed.
• Photocopy of UB92 may be sent.
• Medical records to support appeal.
