Where to Mail Your Cost Report

Your Medicare Cost Report should be sent directly to your Regional CMS Fiscal Intermediary.

National Government Services, Inc (NGS) provides Fiscal Intermediary services to:

Hospitals, Skilled Nursing Facilities, Outpatient Physical Therapy and Comprehensive Outpatient Rehabilitation Facilities, End Stage Renal Disease Facilities, Rural Health Clinics and Critical Access Hospitals in the following states and territories:
Virginia, West Virginia, Michigan, Wisconsin, California, Nevada, Hawaii, Guam, American Samoa and Northern Mariana Islands;

Home Health and Hospice providers in:

Wisconsin, Michigan, New York, New Jersey, Minnesota, Puerto Rico, Virgin Islands, Alaska, California, Nevada, Oregon, Washington, Idaho, Arizona, American Samoa, Guam and Northern Mariana Islands.

Federally Qualified Health Centers

Submit all Federally Qualified Health Center (FQHC) Cost reports to the Milwaukee office with the exception of FQHC's located in California, Nevada, Hawaii and the Pacific Rim. Those located within the west coast offices should send your Cost Report to the Camarillo office.
