The Provider Enrollment Department for National Government Services, Inc processes the CMS-855A application only (applicable to enrolling facilities or changes of information pertaining to an entity that has been previously enrolled into the Medicare A program). The CMS-855I and CMS-855R should be forwarded to your Part B Carrier .
