Change Requests to Existing User ID's

Requests to update "Existing" Direct Data Entry (DDE) user ID's, must be submitted on your company letterhead/fax cover sheet. All of the following information MUST be included:

  • The DATE of your request
  • The Existing DDE User ID(s) that require an update
  • The Medicare Provider Number(s) you wish to add/delete/change to the existing user ID(s)

To assist UGS in returning the completed Ids to your attention, please provide the following information:

  • Contact Name
  • Contact Title
  • Contact Mailing Address
  • Contact Telephone Number (including extension)
  • Contact Fax Number
  • Contact E-mail Address
When your request letter/fax has been completed, please fax the completed DDE User ID Accountability Agreementword and your request letter/fax to the appropriate UGS Service Area for processing.

PLEASE DO NOT CALL FOR STATUS UNLESS IT HAS BEEN MORE THAN (10) BUSINESS DAYS FROM THE DATE YOUR REQUEST WAS FAXED TO UGS. STATUS CALLS AND/OR FAILURE TO INCLUDE ANY OF THE ABOVE REQUESTED INFORMATION WILL DELAY YOUR REQUEST FURTHER.
