• EDI Enrollment Form
  This form is required to be signed and submitted by all new providers who intend to engage in the electronic submission of claims to NGS. NGS EDI will initiate your electronic setup after receiving a fax copy of this form and the Provider - Smart Transfer Information Sheet. We must receive ALL 3 pages or your request will not be processed.
• DDE User ID Accountability Agreement
  This form is required to be signed and submitted for each individual user who intends to access the Fiscal Intermediary Standard System (FISS). NGS EDI will initiate your electronic DDE User ID setup after receiving a fax copy of this form.
