Effective April 1, 2008, National Government Services added the article listed below as an attachment to the Local Coverage Determination (LCD) for Coverage of Drugs and Biologicals for Label and Off-Label Uses (LCD ID #L25820). The article can be found on the CMS Coverage Database and can be accessed on the National Government Services Web site by using the search function or by clicking on the links at the end of the LCD on the National Government Services Web site. This article defines current coverage by National Government Services. The article will be updated in the event of changes in labeling for this drug, or if off-label use is approved by the contractor.
